This MRFF Project has seen the drafting of an in-hospital intervention for all patients admitted to hospital (for any reason) who have Type II Diabetes. Potential healthcare issues and factors to be addressed by the intervention were identified by reviewing the data of all patients with Type II Diabetes admitted to the Lyell McEwin Hospital over 2 years. The elements of the most effective models of integrated care for Type II Diabetes were also identified by a rigorous review of current evidence.
Together, this information was used to develop the draft intervention. Further input into the intervention design will be sought from patients, Aboriginal and Torres Strait Islanders. Aboriginal Health professionals, allied health professionals, hospital medical professionals and General Practitioners prior to finalising the design for in-hospital testing.
A model to assess the cost-effectiveness of the intervention has also been developed and following stakeholder input the intervention will be refined and made available for testing in the hospital setting.
Further MRFF funding will see this project extended into 2019/ 2020.
